Click on the links below to access and fill out the Office Policies, Telemedicine Consent, and Intake forms, and ROI (if applicable) and email them to me at merkorn@comcast.net prior to our first appointment. These help me get to know you better and also to explain office policies, etc. If there is someone with whom you think it would be useful for me to speak or collaborate, please sign separate ROI (Release of Information) documents as well. These can often be for significant others, family members, and other health care providers such as your doctor or psychiatrist. Thank you.
